Neonatal Gastrointestinal Perforation

Abstract

|
Perforations of the gastrointestinal (GI) tract in the neonatal period have been associated with a very poor prognosis. Therefore management of the neonate with GI perforation continues to challenge pediatric surgeons. Generally two major
categories are
encountered: idiopathic and known cause type. The aim of this study is to establish a plan of management in nonates with GI perforation. We retrospectively reviewed 31 neonates with GI perforation who were treated in our hospital from January,
1991
to
December, 1995, The overall mortality was 45.2%. The highest mortality(75%) was associated wit gastric perforation. Colonic perforation carried the lowest risk of mortality(28.6%). The mortality rate from small bowel perforation was 47.1%.
Prematurity
and low birth weight were significant factors increasing the mortality rate. The predominant cause was necrotizing enterocolitis(48.4%). The most common site of perforation was the ileum(54.8%). In conclusion early diagnosis and intensive care is
very
important to increase the survival rate in neonates with GI perforation.
